1) Code of Conduct

I confirm that I have read the CPGSL Code of Conduct and understand that I must agree and conform to the league rules and sportsmanship policies of the Crown Point Girls Softball League. I promise to do my best to conduct myself by the Code of Conduct and will guide my child(ren) accordingly.

 

 

2) Injury Release

I agree to hold harmless the Crown Point Girls Softball League, its board and coaches from any action in law or equity arising against them because of damage and/or injury to my child(ren) or others while engaged in practices, games or other league-sponsored events.

 

 

3) Photo and/or Info Release

I agree that photos of my child(ren), taken in the course of participation in CPGSL activities, may be published for use on the Crown Point Girls Softball League website. Also, if I am selected to coach a team, I understand that my name, phone number and email address will be available to the public on the CPGSL website.
